Citation:  1.) The Distinguished Flying Cross for heroism while participating in aerial flight evidenced by voluntary actions above and beyond the call of duty. Chief Warrant Officer Phelps distinguished himself by exceptionally valorous actons while serving as an aircraft commander of a UH-1D helicopter. Chief Warrant Officer Phelps demonstrated a deep devotion to duty and a desire to complete the mission, although his aircraft was disabled by the enemy. Upon take-off from a landing zone where his flight had been rocketed and mortared, his aircraft received three hits from enemy ground fire rendering critical flight instruments inoperative. Upon a quick assessment of damage sustained, he elected to continue the mission. His complete disregard for personal safety was instrumental in the completion of the mission, although he was continually subjected to hostile fire throughout the day. His actions were in keeping with the highest traditions of the military service and reflect great credit upon himself, his unit, and the United States Army.
